Option 3: Add It as a Popup or Slide-In
If you’d rather show the form in a popup or floating button:
Copy the provided script code.
In Shopify Admin → Online Store → Themes → Edit Code.
Go to theme.liquid (or footer.liquid), before the closing tag.
Paste the script there.
Supply Drop - Box Information Form
Save and test your store — your survey will now appear as a popup or floating form.
